Intensive Case Management Program
The Next Step Housing Connection (NSHC) provides intensive case management housing support services to individuals (aged 16+) or families facing or experiencing homelessness in Hastings and Prince Edward counties through the Intensive Case Management (ICM) program.
An Intensive Case Manager meets with eligible clients to assess their immediate housing needs and develops an individualized housing plan tailored to each client’s needs. The Intensive Case Manager will assist with the housing search process (including contacting landlords, viewings, and negotiations) and can help with completing various housing applications (including applications to subsidized housing programs) as well as assistance applying for various forms of financial support. The Intensive Case Manager will also provide ongoing housing counselling, education and information (including budgeting, life and housing retention skills, and information on the Residential Tenancies Act) to ensure that housing can be maintained. Once housing is secured, stabilization and support services are provided (including mediation support between tenants and landlords) and the Intensive Case Manager can refer clients to other community resources that may be needed. Services are provided at our Belleville, Quinte West, or Picton office locations or virtually.
Who is eligible for the Intensive Case Management Program?
In order to qualify for the Intensive Case Management program clients must meet at least three of the following criteria:
- Be homeless or be at risk of becoming homeless within 2 weeks
- Be actively searching for housing but have been unsuccessful
- Have a mental illness or substance use concern
- Be a victim of domestic violence
- Be living with a disability or communication barrier
- Lack other community support and/or knowledge of local resources
- Have limited social supports (family, friends, etc.)
- Be a voluntary and willing participant
